Who operates this website

Spectrum Health Inc. is the private CRNA practice of Ken Bartman, CRNA, MSN, serving Chicagoland. Questions about this policy may be sent to kenbartman@yahoo.com.

The inquiry form may ask for your name, organization, email address, phone number, professional role, facility information, coverage needs, approximate dates and message. Required and optional fields are identified on the form.

The form opens your own email application with the information you entered. The website itself does not store the completed inquiry in a Spectrum Health database. Your message is transmitted only when you choose to send it through your email provider.
